docs: update phase and unblocked roadmap items

Updated the project documentation to indicate that Phase 24 is complete and Phase 25 (Game-Over Flow) is currently active. Also migrated the active roadmap details from FUTURE.md to STATE_OF_THE_PROJECT.md as they are now unblocked by earlier foundational layers. Affected files: AGENT_ONBOARDING.md, CLAUDE.md, GEMINI.md, FUTURE.md, STATE_OF_THE_PROJECT.md, and README.md.
