Rename replaces too much

Renaming a ticket will also replace all occurrences of the old ticket ID with the new one within the notes file. This could cause inadvertent replacements for short or generic ticket names.

Solution: Replace the ticket ID only in the notes file's title, leaving all other mentions untouched.
