I needed to use the name "footnote" as new button in the toolbar instead of "phrase" to get it running
added js as minified file, using https://minify-js.com/

Hi,
I used your medium editor addon in my project, but I did not get your example code running. By try and error I found out, that the button name was incorrect. Here is a corrected readme and I added a minified js file for conveniance.
