GC outline feedback bug fixes: Hello! every turn, Providing draft

This fixes two bugs:
Hello! every turn:

• Implements a run tracking approach at mode layer to inform gc call if this is the first time user is calling or if user is returning.

Providing draft:

• improves instructions in gc config.

Also returns doc agents gc helper code to default for draft outline mode. Decoupling to be fixed in separate task.
