HLS stream stuck in preparing state

When starting playback of this stream [1], the state changes from idle -> preparing and stays there. No error is reported. The same stream works in Android MediaPlayer.

Here the log stops and ExoPlayer doesn't seem to do anything more.

This video plays well in Quicktime, in the Android MediaPlayer as well as in the martinbonnin-fork of ExoPlayer.
Tested on Android 4.4.4, Samsung S3 LTE (gt-i9305), official firmware.
As well as Android 4.3.1, Samsung S3, official firmware.

Edit: tested using the dev-hls branch, latest version, 60d162d

There are two problems here:

• The error doesn't get propagated properly.
• The player adds a spurious "/" into the segment URLs. This causes requests to fail with a 403 error.

Will push a fix.

There's actually a third (far more minor) problem, which is the spurious P->I->P state transition. I'll fix that today.
