Improved support for Windows

Support Cloudstate protocol 0.2 — Actions, Event Sourced Entities, and Replicated (CRDT) Entities.

Update failure handling:

• fix reporting of failures and unexpected exceptions to the proxy
• if context.fail is called then no events are applied for the current command
