Implements Stark-level execution-engine immutability (LEGACY_DB_LOOP vs TEMPORAL_WORKFLOW), engine-scoped resume ACK semantics, and reconciler re-arm behavior. Includes DB migrations (20260110_* + 20260111_*), and unit tests.
