Maintenance: update PR template & Maintainers roster

Summary

This is a catch-all issue for two separate items related to governance:

• we should update the PR template to include the word closes next to the issue number for better issue linking
• we should update the maintainers roster to reflect current active maintainers

Why is this needed?

So that customers and contributors can have a better experience when opening PRs and when understanding who they're interacting with.
