Command palette: Add `rename` for renaming the document title

What problem does this address?

It's very hard to rename a title in the page editor, especially if the template does not has a page title included.
Users open the command palette, because the click on the title, expecting it to rename it there. (see video in #70968)

What is your proposed solution?

Add rename and title commands to the command palette for renaming the document title.

I was trying to implement a Rename command for the command palette, but couldn’t find a public API to directly trigger the rename modal.

Upon investigating, I found that the modal is indeed invoked from within the post-actions component:

From what I observed, when action.id === 'rename-post', the rename modal is triggered and it all depends on local state setActiveModalAction(). However, I couldn’t find any exposed interface API in openModal() that allows this modal to be triggered programmatically outside of that context

Please let me know if I missed something.
