Add error page for enhanced reporting and user experience

Description

adding error page for easier error reporting and improved user experience
the design is intentionally chosen to encourage reporting the issue by placing the "report issue" button in the most prominent place

The report button will use existing functionality to create a pre filled issue on our github. relying on the mechanism added by @Just-Insane
