Add heatmaps support for patient-level features

Current heatmaps command just works for models trained with tile-level features. The next step is to add support for slide or patient-level features, taking into account the different encoding approaches of the supported models. For COBRA, EAGLE and CHIEF, the implementation can be done similar as here, but not for the other models.
