Both forms serve the same purpose, so you can choose one of the two. The form provided by UT JAMP has all the JFD information pre-filled, but it currently only offers a professor as an evaluator option (we are working to fix this). If someone other than a professor is completing your evaluation form, you will need to use the general JAMP form and fill in the JFD information manually.
