Venture Capital and Entrepreneurship in Heart Failure: A Review for Cardiologists - Journal of Cardiac Failure - Intersections
Collaboration between cardiologists, investigators, entrepreneurs, and investors is critical to addressing unmet clinical needs. This manuscript describes the significant impact of venture capital and entrepreneurship on advancing heart failure (HF) care, and how this ecosystem supports the development of novel diagnostics and therapeutics. The article provides an overview of the venture capital ecosystem and includes case studies within the medtech and biotech sectors, including the development of mavacamten (MyoKardia) and implantable hemodynamic monitoring (CardioMEMS), that illustrate successful collaborations that have improved HF patient care.

Mechanical Circulatory Support Management: A Current Knowledge Deficit in Critical Care Trainees - Journal of Cardiac Failure - Intersections
Over the past decade, there has been a significant increase in the use of mechanical circulatory support (MCS) devices for the management of cardiogenic shock, including extracorporeal membrane oxygenation (ECMO), percutaneous ventricular assist devices (pVADs), and durable left ventricular assist devices (LVADs).1,2 Despite this increased use, most critical-care programs report that their fellows are not adequately prepared to manage MCS devices independently upon graduation.3 Clinical experience is essential for mastery of MCS devices, but variability in clinical exposure by institution has led to a need for supplementary education outside of the intensive care unit to enhance critical-care trainees’ ability to manage patients supported by these devices.

Despite the increased use of veno-arterial extracorporeal membrane oxygenation (V-A ECMO) for cardiogenic shock (CS), mortality rates remain high, at up to 60%.1,2 Studies have identified an association between atrial fibrillation (AF) and mortality in patients supported with veno-venous ECMO,3 but little is known about the frequency or impact of atrial fibrillation (AF) on the outcomes of patients in cardiogenic shock (CS) on peripheral V-A ECMO (pV-A ECMO).

Heart failure (HF) remains a major public health challenge with a significant socioeconomic burden.1 The rationale for introducing beta-blockers in the management of chronic HF was consolidated by the works of Bristow et al, beginning in the 1980s.2–4 Since then, beta-blockers have become a cornerstone in managing chronic HF, offering substantial benefits in decreasing morbidity, mortality, and hospitalization rates through their effect on left ventricular (LV) function and remodeling.5,6 After the approval of metoprolol in the 1970s, 4 different metoprolol formulations have been produced based on different pharmacological properties: tartrate, succinate, fumarate, and hydrochloride.

Heart failure with reduced ejection fraction (HFrEF) is a leading global public health problem with an estimated 5-year mortality rate of 50%.1 Guideline-directed medical therapy (GDMT), consisting of beta-blockers (BBs), angiotensin-converting enzyme inhibitors (ACEis), angiotensin receptor blockers (ARBs), or angiotensin receptor-neprilysin inhibitors (ARNis), mineralocorticoid receptor antagonists (MRAs), and sodium glucose cotransporter 2 inhibitors (SGLT2is), leads to a 73% relative reduction in mortality rates at 2 years for patients with HFrEF,2 yet it remains underused.

Digoxin Use and Post-LVAD Functional Capacity, Quality of Life, and Survival - Journal of Cardiac Failure - Intersections
Digoxin, a medication with inotropic and chronotropic properties, is commonly prescribed for patients with advanced heart failure (HF); however, the data supporting its use in these patients remain controversial. The landmark Digitalis Investigation Group (DIG) trial found that although digoxin did not reduce mortality rates in patients with HF, it did reduce the rate of hospitalization in these patients.1 Previous smaller studies have shown that withdrawal of digoxin in patients with HF may lead to worsening functional status, quality of life, and left ventricular ejection fraction.

Despite declining age-standardized death rates for ischemic heart disease over the past several decades, the burden of disease remains substantial, contributing to a growing population with ischemic cardiomyopathy (ICM).1 Definitive treatment options for this population have been fraught by controversy and mostly focused on combinations of optimal medical therapies (OMT), percutaneous interventions, and surgical revascularization. With significant implications for survival, quality of life, and health care costs, effective and efficient approaches to dealing with ICM are needed.
