Welcome to Bridge the Gap Physical Therapy! This questionnaire is for Athletes and Clients experiencing pain or injury and are interested in a REMOTE service to build back to where you want to be. The Bridge the Gap Athlete Centered 3 Step Process 🔹Assess: Individual pains and history, injury behavior, training and function limitations, individual strengths, limiting factors, skills, and goals. 🔹Customize: Begin with a 4-12week Strength and Fitness based Rehab plan to take you from where you are now to the place you want to be (this can include hitting lifetime PRs) 🔹Integrate: Workout, learn to self-manage, develop a strong mentality against pain and injury, reduce future injury risk with this holistic approach to Fitness and Rehab All coaching is carried out by Dr. Trevor Souik, DPT, CSCS, BRM Dr. Trevor is a Physical Therapist who specializes in working with Strength and Performance Athletes We look forward to hitting new PRs together!
