U18S CONSENT FORM FOR MARY STREET GIG
The following requests and restrictions are in place for your safety. Ideally under 18s would attend with an adult (over 21) with a ratio of 2 young people to one adult. Under 18s who wish to attend the concert on Saturday 28th February unaccompanied need to provide contact details of their parent or carer. Anyone under 16 cannot attend unaccompanied by an adult, over 21. These details will be verified, and if confirmed you will be added to a guest list for security staff on the day so you can be admitted. If we are unable to verify these details you will not be able to attend the concert unaccompanied. We don’t want to turn anyone away so please provide answers to all of the following questions. Please provide these details by 16/02/26. The venue will be closed by 10:30pm at the latest. Be aware there will be bag searches in operation at the venue. We also operate a no readmission policy for any under 18s who wish to go outside but do not stay within the cordoned area.
