Blood Drive Pledge Form
We would like to host a blood drive on 2/14 in conjunction with New York Blood Center. To be ensure our event will be successful, we need at least 70 people to volunteer and pledge to donate ahead of time. Donating blood is easy. It takes only about one hour of your time and each donation can save up to three lives. Your donation can help cancer patients, accident victims, people undergoing surgery, women during childbirth, people with sickle cell disease and many more. We want to do our part in helping to save lives in our community. Will you help us? Please fill out the pledge form if you are interested in being a blood donor and we will contact you to confirm the date and time of our blood drive once we have enough pledges. Qualifications to donate: 16+ years old (16 w/ parent consent) 110+lbs General good health Valid ID Thank you for your support!
