Add page via form not working

Hiya, 

I am trying to create a page via a form template, however when I click on the button to do so, a blue loading wheel appears for a few seconds, then disappears - no page is created.

I can confirm that the macro is set up correctly, as we have used this template and button many times up until now. I have confirmed that this is happening on multiple pages using the same macro. I have also tried clearing my cache and using an incognito window. 

Is this a bug, or how do I get around fixing it, please?

It sounds like you covered some ground already, which is great.

There's a number of possible causes for the behavior you reported:

• browser extensions causing conflict with the Create from Template button
• network calls being blocked by a VPN, Firewall or other
• a temporary incident affecting your Confluence site

It's difficult to determine exactly what the root cause is without a closer look, so the best option now is to open a Support ticket and have our Team assist you.
