About you and your circumstances

A) By submitting this form you confirm that:

you are aged 18 or over;
you are single and/or divorced or permanently separated from your spouse or partner;
you are living separately from your former spouse or any former partner;
you will co-operate with us in verifying your identity and place of residence, which co-operation may extend to the provision of further identification and proof of residence if we cannot verify this information using our usual methods;
you will agree to us conducting a background check using a third-party agency, and if and when requested will provide all necessary details requested by the third party to conduct the checks properly;
you do not have any criminal convictions or cautions;
you will inform by email as soon as practicable if there is a material change in your relationship status and/or circumstances. i.e., that you are no longer single or other information disclosed to us has changed;
you are not an employee of or associated with or acting on behalf of another relationship, introduction or dating agency and/or are not misrepresenting yourself to Project 143 with the objective of gaining insight to or knowledge of our methodologies to enhance your or your associate or employee’s business or to set up and start an agency of this kind of your own, or for any other reason;
all information you supply to us about you and your circumstances is accurate, up-to-date and not misleading and you have not withheld any relevant information.

Data Protection & Confidentiality

B) You acknowledge and agree that:

all personal information will be collected, used and held in accordance with the provisions of the Data Protection Act 2018 and your rights under that regulation;
we take our data protection obligations very seriously. We will not share your personal information with third parties without your prior consent. We will not reproduce or publish your personal information online. Everything we do is highly confidential and your personal details will only be shared with 'potential matches' by e-mail. This includes non-sensitive and generic (non-identifying) information about you as follows: (i) your first name, age, occupation, location, religious/cultural background, height, number of prior marriages, number of children, smoking habits and diet preferences; (ii) your photo; and (iii) a summary of you based on the information provided in this Questionnaire. Other personal information will not be shared (except where authorised by you or with your prior consent)
we may use your personal information to undertake our general business activities, process payments by you, and inform you of new products and services available from us. You may request that we stop sending you marketing information at any time; and
