Care Coordination Advocate

The Care Coordination Advocate supports participants with their safety and wellbeing goals and ensuring continuity with AWARE services. The Care Coordination Advocate serves two primary roles (1) case management for AWARE participants, and (2) coordination of care between participants, AWARE staff and our external partners.
Streamlining communication flow, and the participant’s experience, between shelter, housing and legal staff is an essential role of the Care Coordination Advocate.

RESPONSIBILITIES:
- Establish supportive, survivor-centered relationships with AWARE participants to increase resilience to everyday challenges, capacity to manage overwhelm and foster hope for the future. Incorporate trauma-informed practices that prioritize respect and understanding of participants’ specific circumstances and self-identified needs.
- Greet and welcome walk-in community members as needed.
- Provide phone coverage including crisis line safety response.
- Understand and practice safety-planning with residents experiencing current and historic incidents of domestic violence and sexual assault.
- Work closely with the Shelter Operations Manager to seamlessly bridge advocacy services and shelter living.
- Continuously bolster healthy relationships between participants and shelter staff.
- Foster a cohesive team with fluid communication. Develop and maintain communication procedures that safeguard participant confidentiality and ensure participant needs and requests are understood across departments.
- Provide case management for shelter residents by facilitating resident progress through AWARE’s case management guidelines.
- Establish weekly (or more frequent) meetings with each shelter resident to maintain progress toward defined goals, successful engagement in AWARE services and completion of tasks.
- Complete assessment of participant’s safety needs that includes immediate as well as long-term risk factors to develop a comprehensive safety plan. Provide follow-up as needed.
- Assist AWARE participants to identify goals and objectives needed to obtain safety, self-sufficiency and well-being. Monitor and document client progress toward goals and objectives, initiate assistance to clients for completion of goals and objectives and recommend revisions to services or objectives as needed.
- Acquire basic knowledge of DV/SA-related criminal and civil laws, rights of victims etc. and be able to communicate that information to participants. Be prepared to assist any survivor with emergency Protection Order advocacy. Provide referrals for participants seeking civil/criminal legal assistance.
- Provide assistance/accompaniment for families working with the Office of Children’s Services. Prioritize referrals from OCS for safety planning and trauma-informed interventions.
- Provide linkages between AWARE and other service providers by coordinating services, participating in case planning meetings and providing systems advocacy on behalf of AWARE participants. Provide appropriate referrals, and necessary support when connecting participants with community partner agencies. Maintain positive relationships with these partners to accommodate eligible referrals and ensure referrals are productive.
- Develop expertise and implement processes for accessing behavioral health treatment resources and be able to guide participants through requirements.
- Conduct shelter intakes and exits and ensure coordination with shelter staff to ease the impact of communal living.
- Assist hiring supervisors in training, reinforcing and mentoring AWARE staff. Support staff competency with crisis intervention, safety planning, advocacy, education and support to victims of domestic violence and/or sexual assault (DV/SA) and their children.
ALL STAFF RESPONSIBILITIES:
- Agreement with and adherence to AWARE’s foundational practices of service and welcome for survivors. This includes our “screen in, not out” practice as well as non-discrimination for any survivor in a protected class and/or experiencing behavioral health issues.
- Demonstrate trauma-informed service readiness, including recognition of common trauma responses and their presentation (behaviors, words, tone of voice, etc.), and respond appropriately.
- Demonstrate an ability to self-regulate and an awareness of internal reactions to trauma. Practice regular self and community care to mitigate secondary trauma
- Provide crisis intervention, safety planning, advocacy, education and support to victims of domestic violence and/or sexual assault (DV/SA) and their children.
- Ensure compliance with all state and federal regulations within shelter and community victim services.
- Perform all record-keeping activities associated with client contact as required for AWARE data collection.
- Maintain cultural sensitivity and responsiveness at all times.
- Report all cases of suspected abuse or neglect of children, or vulnerable adults to the State of Alaska Office of Children’s Services, Adult Protective Services, and law enforcement as required.
- Participate in the performance of other tasks essential to program and household operation and maintenance including shelter shift coverage.
- Meet regularly with supervisor for supervision and support.
- Attend meetings and staff trainings.
- Comply with AWARE Personnel and Administrative Policies.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Must pass AWARE background check.
- Valid AK driver’s license.
- Must have or acquire CPR and First Aid Certification.
- Ability to complete basic, general household repairs and preventative maintenance.
- Ability to work independently.
- Must have excellent written communication skills.
- Must have basic computer skills.
- Must have the ability to remain objective and to perform under stressful conditions.
- Must have the ability to understand and communicate with people of different socioeconomic and cultural backgrounds.

Care & Crisis Advocate

Our Care & Crisis Advocates provide safety and support for survivors of domestic and sexual violence. They work with residents in our emergency safe shelter and answer our 24-hour careline. Training is provided!
We’re hiring both full and part-time positions for overnight weekdays and days during the weekend.

DUTIES AND RESPONSIBILITIES:
- Greet and welcome walk-in community members as needed.
- To provide exceptional crisis-line response, prioritize crisis intervention skill building.
- Understand and practice safety-planning with residents experiencing current and historic incidents of domestic violence and sexual assault.
- Conduct shelter intakes and exits, and ensure appropriate information is gathered for handoff to direct service staff.
- Assess all shelter participants’ situations to determine the urgency of their need and the nature of services or interventions required. Follow handover process to AWARE’s direct service staff (housing, legal and safety) prioritizing immediate needs when appropriate.
- Complete assessment of participant’s safety needs that includes immediate as well as long-term risk factors and provide effective referrals to AWARE’s care coordination advocates. Provide follow-up and maintenance of safety planning, as requested and in coordination with AWARE direct service staff.
- Acquire basic knowledge of DV/SA-related criminal and civil laws, rights of victims etc. and be able to communicate appropriate information to participants and provide effective referrals to AWARE civil and criminal legal assistance.
- Acquire basic knowledge of housing assistance resources and provide effective referrals to internal and external housing resources.
- Acquire basic knowledge of common mental health issues and substance use disorders and be able to communicate that information to participants. Provide appropriate external referrals for participants seeking more intensive behavioral health and other resource assistance.
- Model problem resolution skills for residents to solve roommate/communal living issues and facilitate resolutions for critical issues.
- Recognize emergencies and act to ensure participant and staff safety. Use de-escalation techniques to defuse tense situations. If needed, request law enforcement assistance and be able to provide critical information to law enforcement. Ensure notification of the manager-on-call and documentation for staff.
- Conduct safety procedures according to policy and procedures.
- Responsibility for shelter cleanliness, stocking and maintenance including:
- Perform basic cleaning tasks and maintain the upkeep and cleanliness of shelter.
- Complete chore and laundry responsibilities to ensure cleanliness of shelter according to checklist. When appropriate, involve residents in communal chores.
- Replenish shelter food and supplies. Shelve basement donations and report shortages per protocol.
- Conduct room checks and regular maintenance of common spaces.
- Report maintenance issues per protocol.
- Assist in the training of new relief, permanent or volunteer shelter staff, as requested.
All Staff Responsibilities

Relief Shelter Advocate

DUTIES AND RESPONSIBILITIES:
- Demonstrate familiarity with AWARE’s helping values and practices.
- Have working knowledge of DV/SA including: dynamics of battering, sexual assault, chemical dependency, lethality and safety planning.
- Be able to quickly assess and respond to crisis/emergency situation or person. Be able to use de-escalation strategies such as isolation. Work with lead advocate and/or director to ensure appropriate response including request Law Enforcement assistance and be able to provide critical information to Law Enforcement. Follow all directives pertaining to the situation including notifying the shelter supervisor.
- Practice positive role modeling that promotes strengthening/supporting parent-child relationship in ES. Support parent to ensure children are able to comply with safety and confidentiality for all ES residents/participants.
- Complete assessment to determine participant’s immediate safety needs and assist with developing a response to the safety issues.
- Provide trauma-informed victim advocacy, this includes: crisis intervention, answer 24-hour Crisis Line, assist with service delivery.
- Be able to provide victim services to individual with active substance misuse and/or mental health crisis.
- Screen participants for admittance to the shelter and complete ES program enrollment as appropriate. Provide new residents with an orientation to ES. Refer all individuals not meeting our admittance criteria to the proper community agency.
- Perform security checks of the shelter facility at least once each shift. Report security/ safety issues to manager.
- Provide a verbal account of shift activities at shift change, this includes security alert and maintenance issues. Ensure all required record-keeping duties are completed prior to end of shift.
- Perform general administrative functions such as log communication with participants and other record keeping activities associated with program participant contact; or as required for data collection and inter staff communication.
- Participate in the performance of other tasks essential to program and household operation and maintenance.
OTHER STAFF TASKS:
- Request law enforcement officers to perform welfare checks on participants we suspect to be in immediate danger, as appropriate.
- Report cases of suspected abuse or neglect of children or vulnerable adults to the Alaska Office of Children’s Services or Adult Protective Services, and the Juneau Police Department as required. ∙ Meet regularly with supervisor for supervision and support.
- Required to attend a minimum of one advocacy meeting per month.
- Required to work a minimum of one shift per month.
- Attend a minimum of one all-staff training per year & meet continuing education requirements.
- Comply with AWARE Personnel and Administrative Policies.
MINIMUM QUALIFICATIONS:
- Must pass AWARE background check.
- Must have high school diploma/GED
- Must have or acquire CPR and First Aid Certifications.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Must have a thorough understanding of domestic violence, sexual assault and the needs of victims or the willingness to learn and listen.
- Must have the ability to remain calm and objective and to perform under stressful conditions.
- Must have the ability to understand and communicate with people of different socioeconomic and cultural backgrounds.
- Must have basic computer skills or the ability to learn.
- Must maintain confidentiality.
- Must support AWARE’s mission and philosophy
