Are you being referred to AMS Cardiology by another provider?
If you were recently in a hospital other than Abington Memorial or Abington Lansdale, and you’re coming for a follow-up, please also request your hospital records.
Complete our “Records Release Form” and give it to your doctor or hospital.
Have your records faxed to us at 215-376-1705.
